Long term support
Rapid Improvement Supported Living offers high quality supported living accommodation for people who require long term support with a variety of diagnosis, including learning disabilities, autism, mental health, physical disabilities, acquired brain injuries, behaviours that are deemed to be challenging, forensic support and EUPD, as well as those with a dual diagnosis.
Our model of support
Our 24 hour supported living services offer homely and supportive environments that are focussed on the individual needs and aspirations of the people we support. Supported living pathways – which include support living, supported living plus and supported living extra services – enable us to work towards increasing independence with the ultimate goal of more independent living.
The importance of person-centred care
The ultimate goal of Rapid Improvement Supported Living is to provide support that can be reduced over time as independence increases. However, we also recognise that, for some, long term support will be required. In these circumstances, we offer long term support that ensures continuity of care within a stable, familiar setting.
Our care plans are tailored to the individual needs of every person we support. We work with the individual, their family and their wider support team (where applicable) to ensure that the care we provide meets that changing needs of every individual.
